varicocele

DEFINITION: For varicocele is defined as a dilatation of the pampiniform venous plexus. It manifests as a worm-like appearance of swelling , Expandable to coughing , at the level of the scrotum ( much more frequent on the left).

ETIOLOGY: It affects males , and recognizes a genetic predisposition.

SYMPTOMS: The disease can be asymptomatic , in some cases can give a feeling of weight or pain at the level of the scrotum.

DIAGNOSIS: The diagnosis is clinical (Visit Specialist) in some cases completed dall'ecocolordoppler and scrotal ultrasound.

TREATMENT: If asymptomatic in the absence of infertility problems can be useful only a jockstrap. In the presence of infertility problems , confirmed by a semen analysis , can benefit from the surgery. It consists in the ligation and excision of spermatic vein ligation or selective pampiniform plexus. Alternatively, it is also proposed a sclerotherapy , in our experience with poor results.

COMPLICATIONS: The disease generally has no complications but only the effect of increasing the testicular temperature and then the male fertility. The only complication of the surgery is the postoperative hydrocele , a collection of fluid in the scrotum that resolves spontaneously or after some aspirations. When varicocele occurs on the right side instead be carefully investigated the abdomen because it could be indicator of disease compressive (tumors).

SURGERY FOR VARICOCELE

Many studies have correlated male infertility with varicocele.

Indications for surgery

The indication for intervention is in the presence of a varicocele or symptomatic of a spermiogram insufficient in a pair desirous of a pregnancy.

Results

The results vary depending on the accuracy of the diagnosis since the symptoms accused can also depend on type of orthopedic disorders (back pain). The results with regard to the fertility are very good , but not absolute.

Depending on the indication you can make the pampiniform plexus ligation or ligation by removal of part of the spermatic vein ( modified according to Palomo). The first is executable under local anesthesia , while the second requires spinal anesthesia. We do not carry out sclerotherapy because of the discouraging results in the personal.
